All of your responses will remain confidential and any identifying information provided will be removed to protect your anonymity. All personal information is being collected under authority of section 26(e) of the Freedom of Information and Protection of Privacy Act (FIPPA). 

Survey results, including written responses, will be used for annual program evaluation and anonymized comments may be utilized in reports.
