The survey focuses on five key domains. We acknowledge these domains are cross-cutting and may intersect with other core services such as housing and health care.


1. Communication, Technology and Language: Includes programs and services that support communication and access to information and resources, such as assistance with form completion, information and referral services, library services, literacy/digital literacy training, technology provision or subsidies and translation.


2. Food Security: Includes grocery shopping, meal delivery, food banks, subsidies and other programs and services that support reliable and affordable access to safe, nutritious and culturally appropriate food.


3. Home Supports: Includes non-medical services such as light housekeeping, home maintenance and garden care, pet care and other activities that support aging in a place of choice.


4. Social and Activity Programs: Includes the full spectrum of cultural, educational, fitness, peer relations, recreational and social programs that support active engagement and connections for mental and physical health.


5. Transportation: Includes accessible rides, group outings, shopping rides, volunteer driving for social or medical activities and other options to support community connections and engagement.
